Non-incisional vs. incisional double eyelid surgery

Eyes

The suture method forms the crease with stitches without cutting the skin, so recovery is quick and it is easy to reverse, but the crease can come undone. The incisional method cuts the skin and tidies the tissue, so the crease lasts longer, but the swelling lasts longer too and it is hard to reverse.

01Suture (non-incisional) method

Without cutting the skin, stitches are tied inside the eyelid to create the fold line. Swelling goes down relatively quickly and there is little concern about scarring. If you do not like the result, it is also easy to reverse by releasing the stitches.

On the other hand, the crease can come undone or become shallower over time. This is more likely when the upper eyelid has a lot of fat or the skin sags.

02Incisional method

The skin is cut along the crease line, excess skin, fat and muscle are trimmed, and the line is fixed in place. The crease lasts a long time, and sagging skin can be tidied at the same time.

Swelling lasts longer than with the suture method, and a scar may remain. Once made, the crease is hard to reverse.

03Side by side

Suture method Incisional method
Skin incision None Yes
Main swelling About 1 week 2 to 3 weeks or longer
Crease Can come undone Lasts a long time
Reversing Relatively easy Difficult
Can be combined with Limited Removing sagging skin and fat

04When each method tends to come up

  • Thin skin with little sagging: the suture method is often discussed first.
  • Puffy upper eyelids or sagging skin: the incisional method or fat removal is discussed as well.
  • Weak eye opening: ptosis correction is sometimes discussed together with the double eyelid.

06FAQ

Can I have the suture method if my upper eyelids are puffy?

It is not impossible, but with a lot of fat or sagging skin the crease tends to come undone, so the incisional method or fat removal at the same time is often recommended.

Do the stitches from the suture method stay in my body?

They stay inside the eyelid. If you want to remove the crease or a problem comes up, the stitches can sometimes be taken out.
